The Government Records Access Management Act (GRAMA), Title 63G Chapter 2 of the Utah Code, is the records law for the State of Utah. GRAMA defines what a record is and establishes the criteria for accessing government records. This form is not to request information already available to the public.

Agency Action Review and Appeal Requests
If you are not a current child care provider, use this form to request a review of any decision made or action taken by Child Care Licensing. If you are a current child care provider, please submit your request through your Child Care Licensing Portal. A request for a review or an appeal does not guarantee the action will change. Compliance with rules and requirements is required during the review or appeal process.
