How to Submit Background Check Forms and Fingerprints

Covered individuals are those required to have Child Care Licensing (CCL) background checks

Covered Individuals

The covered individual is to complete the following steps:

First:
Complete a Background Check Form and submit it to the provider.
(For covered individuals younger than 18 years old, this is to be done by a parent or guardian.)

Then:
When instructed by the provider to do so, have Live Scan (electronic) fingerprints done and submitted to the Office of Background Processing. To do this:

  • Take your fingerprint form to a police department, a public safety building, or another Live Scan Fingerprinting Locationopens in a new tab. (Be sure to check the hours of operation for the chosen location.)
  • Pay any additional required fees.
  • Present a valid government-issued photo identification and hand your fingerprint form to the technician capturing your fingerprints.
  • Complete the instructions given to you by the technician.

Providers

The providers is to complete the following steps:

First:
Ensure the Background Check Form is authorized through the Child Care Licensing Portal. Forms are not submitted to the Office of Background Processing until they are authorized. To do this:

Then, when fingerprints are required:

  • Click the "Download Fingerprint Form" button.
  • After the form is downloaded, give the form to the covered individual to print and hand to the technician capturing their fingerprints.
